Clinic Offerings
Medical – Volunteer medical providers (Medical Doctors, Advanced Practice Registered Nurses and Physician Assistants) provide short-term care. Any prescription written must be on the Clinic with a Heart formulary. We do not prescribe narcotics, controlled substances, or anti-psychotics. If a patient does not have a permanent health care provider (medical home), they will be given information about finding a provider at discharge.
Qualifications: Anyone may be seen.
Limitations: Patients may access medical services up to three times in a twelve-month period.
Dental – Volunteer Dentists provide free dental exams which may include x-rays and antibiotics. No procedures are performed on the day of the appointment. Upon discharge, patients will be given a referral to the most appropriate and cost-effective resource to have the needed procedures completed.
Qualifications: Anyone may be seen.
Limitations: There are no limits on the number of dental exams a patient can have.
Chiropractic – Volunteer Chiropractors provide minor adjustments (therapeutic manipulation) for patients.
Qualifications: Anyone may be seen.
Limitations: There are no limits on the number of chiropractic adjustments a patient can have.
Mental Health – Volunteer Counselors will meet with you to explore issues you may be experiencing like depression, anxiety, grief, isolation. This includes impacts from COVID-19, like loss of work or income.
Qualifications: Anyone may be seen.
Limitations: There are no limits on the number of times a patient can come for mental health.
Vision Voucher Program - Vision exams are provided through a Vision Voucher program. You can fill this form out on our website under the Patient Forms.
Qualifications: Patients must not currently have vision insurance, should not have gotten glasses from CWH in the last two years, and should meet income requirements.
Limitations: Patients cannot have vision insurance. A patient may access vision services one time every two years.
Hearing – Volunteer audiology students from the UNL Barkley Center provide hearing screenings. Patients who need follow up will access services through the Barkley Center.
Qualifications: Anyone may be seen.
Limitations: There are no limits on the number of hearing exams a patient can have.
Podiatry – Volunteer podiatrists provide foot care, including care for ingrown toenails, calluses, ulcers, and foot pain.
Qualifications: Anyone may be seen.
Limitations: There are no limits on the number of podiatry exams a patient can have.
Patients are referred to these services after an evaluation at our regular clinics.
Specialty Clinic (Allergy & Asthma, Dermatology, Orthopedic, Pain Management, and Physical Therapy) – Volunteer specialists provide Clinic with Heart patients with specialized health care.
Qualifications:
- Patients must complete the Community Medical Referral process (apply for General Assistance and apply for Medicaid).
- Patients must live in Lancaster County.
- Patients must be uninsured.
Limitations – There are no limits on the number of specialty care appointments [GS3] a patient can have.
Hypertension – Our only chronic care clinic, volunteer Medical providers care for patients whose primary health concerns is high blood pressure.
Qualifications:
- Patients must complete the Community Medical Referral process (apply for General Assistance and apply for Medicaid).
- Patients must live in Lancaster County.
- Patients must be uninsured.
- Patients must not have an established medical home.
- Hgb A1C must be 6.5 or less or if diagnosed with DM2 and controlled on oral agents 8 or less.
- Patients must not be pregnant.
- SBP 140 or more or DBP 90 or more.
Limitations: There are no limits on the number of hypertension appointments a patient can have.
Dental Extractions – Volunteer dental students from UNMC College of Dentistry with oversight by volunteer Dentists (who are adjunct faculty at UNMC) provide simple extractions (for teeth above the gum line)..
Qualifications:
- If patients have a plan to get dentures through UNMC College of Dentistry, they should first have an appointment with the college with a referral to Clinic with a Heart for extractions.
