Telehealth Services

Clinic with a Heart has limited availability for telehealth.  This is a service that allows patients to connect with a provider online rather than in person.  The service is provided during our regular clinic hours.  Here is how the telehealth service works:

1. Call to make an appointment.  Phone lines are open Monday through Thursday after 2:00 p.m.  You can call 402-421-2924.  Please understand that telehealth is not a good option for all conditions.  The person making the appointment will assist you.  When you call, you may be given an in-person visit instead. 
2. If you are given a telehealth visit, it will be on a Thursday evening.  The telehealth coordinator will call you after 4:00 p.m. to make sure the technology needed is working and talk about the next steps.
3. Prior to your visit, you will need to complete a "Telehealth Consent to Treat" form and a health history form.
4. If you are given a visit with a provider,  you will visit them online in a special private room.
